Birth and Death Certificates

Clermont County Public Health issues all Birth Certificates filed in the State of Ohio. To obtain a certified copy of an Ohio Birth Certificate you may order online (NOTE: this service is only available for birth certificates for those born in 1908 and later), or complete the Birth/Death Request form and submit the request to our office. If you are ordering a certificate for a newborn please allow a minimum of two weeks after the child was born before requesting/ordering a Birth Certificate. All Birth Certificate orders are NON-REFUNDABLE.

Typically a request for a vital record can be fulfilled in office in 15-20 minutes. Should you require the certificate to be mailed to you please include a self-addressed stamped envelope so that we may mail the certificate back to you. If an envelope with correct postage is not included with your request your request cannot be fulfilled.

Death Records

All death records are maintained in the county where the death occurred. If the death occurred in Clermont County the certificate will be on file with the Clermont County Public Health. We recommend you contact us before arriving to verify that the certificate is on file.

Death Certificates cannot be ordered online. To obtain a certified copy of a Death Certificate filed in Clermont County please fill the Birth/Death Request form out completely. Typically, a request for a vital record can be fulfilled in office in less than 15 minutes.
